INTERLOCKING NAIL SYSTEM Interlocking Nails are a refinement of the original Kuntscher Nail. An Interlocking Nail is basically an intramedullary pin secured in position by proximal and distal transfixing screws which engage the bone to the nail to provide axial bending and torsional stability. This procedure, in man, generally involves power reaming and insertion by closed technique on a distraction table under fluoroscopic control. The Dueland Interlocking Nail System (IN System), however, needs no extra specialized equipment.
In 1989, R. Tass Dueland, DVM (University of Wisconsin) and associates began investigations to apply this human fracture modality to veterinary orthopedics. These investigations consisted of both mechanical and clinical trials to define the nail and instrumentation requirements. This research provided encouraging clinical results and resulted in the development of the IN System. In comminuted long bone fractures, the IN System is useful due to the mechanical advantage of medullary implantation (i.e. mechanical axis) and prevention of collapse of the fracture by the interlocking effect. Ancillary cerclage wires reduce periosteal stripping and minimize disturbance to blood supply compared to plating. The IN System was developed specifically for the veterinary surgeon to provide a practical surgical procedure requiring minimal ancillary equipment. * No power reaming required to insert the nail In man, fractures of the femur, tibia and humerus are routinely and successfully treated with Interlocking Nails (IN). The advantages of this proven human fracture modality are now available for veterinary orthopedics. It is fast, economical and strong. The IN System is a complete set of surgical instrumentation for implanting interlocking nails in small or younger large animals. The Original Interlocking Nail Systems ™The Model 11-6mm/8mm System is indicated for more extensive shaft fractures, allowing 2 screws to be used in minimal proximal and distal bone. The components of this particular option accommodate the 6mm diameter nail with a 2.7mm size hole and the 8mm diameter nail with a 3.5mm screw hole. Both the 6mm and 8mm nail have hole spacing of 11.1mm. Nails are available in 6 different lengths with either 3 or 4 holes. The model 11-4.0mm/4.7mm is indicated for fracture cases of smaller animals. The components of this particular system accommodate the 4.0mm/4.7mm diameter nail with 2.0 mm size holes with an inter-hole spacing of 11.1mm. The 4.0mm/4.7mm nails are available in 7 different lengths with either 3 or 4 screw holes.
